How to create a report with a date range and subsections.

Oops! Have just found my error. Thought I'd just double check and found my objects on the report were set to Field not Label. All solved now - sorry to have wasted any time!

Interestingly though I can't get it to apply a format to the values that are displayed in the report labels.

I think you need to format it when the Hash Cookie is set / with JS. Also see here:
