Could you export the form's sql using the cloner + dump the used tables (without data)?

Sorry but I am lost :-(
In your NB, Login as globeadmin =>select database tab then click on cloner => Add=> choose form you need to dump. ensure that Dump SQl statement is checked and click RUN.

Here is the dump you have requested, saved in .html ... but impossible to select for attachment :-(
I saved it in .png
I hope you will be able to manage.

I cannot attach .html. So, I saved .png.

Can you also dump the tr_contact table with phpMyAdmin?
