Laying out my form

Hi
I start my form with two very long fields at the top filling most of the width (1st one is a display, and under it is a lookup)
Under that I need to revert back to 4 columns of "regular" fields spaced evenly across the screen
I can't crack it and it's slowing me down a bit.
I wouldn't normally mind (too much) but these are columns of dates that need to be in a particular order for our work, and they just keep trying to go to the right of the long fields at the top).
Anyone got some working html I could put between my long fields and the regular columns.
I know we can use tabs but this is very tightly related info and needs to be visible all at once.
