[URGENT] User is changing and objects not appearing

Hello guys,
I have a following problem, objects are not being added to form and also when I view the form, user changes.. What seems to be going on?

Hi,

Could you press CTRL+SHIFT+G on the form to open the "Objects Grid" form and then take a screenshot?

here it is, I have deleted "new_reconcilation_1" it was just a mock

I figured it out, I was missing placeholder tag for Reconcilation object.
