Main tab not showing in form properties

I've created a new form and when I went to create a new tab the main tab was not there. When I add a new tab and click save the tab I added disappears. Any ideas? I attached the screenshot it would not let me cut and paste it into this message. I'm running version 4.5.

Do you see any objects on your form? Could you create a dump of the form and send it to me?

Yes, there are objects on the form. First Name, Last Name, etc. How do I create a dump of a form?

Under Database/Dropdown, choose Cloner, then "Add", then in "From" lookup, choose you form, then hit Run.

Done, where is the dump? I assume it is a file?
