Access levels - adding forms not possible

Hi,

When I add a new row to the Forms-grid and hit the save button, all the fields (of the new row) are cleared and the new entry is not saved to the database.
Aside from that, I cannot add new rows to add more forms.
You do not have the required permissions to view the files attached to this post.

I believe I have fixed that now - on Github.

Please test it for me.
